Classroom Technology & AV Training Request – For training requests related to: Classroom Technologies, Clickers (polling), Classroom Capture Applications, Multi-Media Authoring Tools, Social Media in the Classroom, Video/Audio/Web Conferencing

Enterprise Applications Request – For requests related to enterprise applications and systems integration with: Banner, APEX, Appworx, Oracle, Red Hat, Hyperion, ImageNow, Meeting Room Manager, LinkUp, Nolij, SeatGen, ACT!, Aces2, Blackboard Connect Integration, Blackboard Transact Integration, Blackboard Learning Management System Integration, Raiser’s Edge, Symplicity, WorkforceNow.

Media Services Request – For requests related to: Class Capture, Event Production, Event Support, Classroom Support, or Media Display.

Network Service Request – For requests related to: Telephony, ActiveSync on Mobile Devices, Enterprise Storage, Email, Account Provisioning, Network Security, Wireless, Life-cycle Management.

Web Service Request – For requests related to: www.nyls.edu, Faculty and Enterprise Blogs, go.nyls.edu, or CityAdmin Document Repository.

Project Charter – For requests that would require services that fall outside of daily operations within the Office of Information Technology. This form serves as the initial needs assessment that is to be completed by the requesting body. From this request, a formalized Project Charter will be developed which will outline a preliminary delineation of roles and responsibilities, project objectives, stakeholders, and authority of the project manager. It serves as a reference of authority for the future of the project.
